In the first quarter of 2025, the team screened 764 patients during 52 visits across five counties. Among those served was Corey, who is well known by the Mobile Health Unit team. He approached the staff at a warming station complaining of shortness of breath and an inability to lie on his back. Nurse Lori Bergland, RN, was familiar with his cardiac history, which includes congestive heart failure. After a brief hospitalization, the team learned that he had been without his medications for nearly two weeks and had no means to pay for them. Lori began working to help him navigate a plan with his
