BSN PROGRAM HEALTH HISTORY WRITE-UP FOR THIS ASSIGNMENT • You will need a family member or friend, preferably someone who is 65 years or older as your volunteer to be your patient BEFORE YOU BEGIN • The terms included here may not be familiar to you. If they are not, look up the terms in your medical dictionary or text glossary. • DO NOT use medical jargon; translate into layman’s terms during. Your interview. • Do not include the personal identifying information of your volunteers. • Review Chapter 4 of Lab Manual Before starting health history: ● Wash hands ● Address client using formal titles such as “Ms., Mrs., Mr. & Last Name”. ● Introduce yourself using your formal title & your role. ● Explain the need for a health history including how long it will take & that all information will be kept confidential.
