Initial Referrals – Initial Eligibility
Reason for a referral: A parent or the school district may ask for a child to be evaluated for a variety of reasons,
such as they are failing, cognitive issues, inability to read, behavior or emotional issues, physical limitations, etc.
Timeline:
30 calendar days 60 calendar days
Referral Receipt of Consent to Evaluation CSE Meeting Program
Referral (PWN) Evaluate Received Initial Eligibility Implementation
Mailed to Parent Determination
With Consent Form Meeting
Once the referral has been received, the Clerical Aide should:
STEP #1 Date stamp in referral from either the parent or the staff member.
STEP #2 Send a copy of the referral to the Chair(s), and the school psychologist.
STEP #3 With approval of the Chair(s), this referral should be put on the PST agenda as a point of
interest for the Committee.
